The shoulder issue surfaced after Djokovic exited Indian Wells in the fourth round against Jack Draper on March 8. He lost 4-6, 6-4, 7-6(5) in a match that lasted over two and a half hours. Organizers confirmed his Miami withdrawal days later, followed by his Monte Carlo pullout on March 26.

Djokovic holds three Madrid titles from 2011, 2016, and 2019. His last clay appearance came at the 2024 Paris Olympics, where he captured gold. The three-time defending champion at the event now targets rhythm on the surface before Roland Garros.

Entry lists finalized with Djokovic at No. 3 seed behind Alcaraz and Sinner. Alexander Zverev, Lorenzo Musetti, and Alex de Minaur complete the top ranks. The field elevates Madrid as a key clay opener.

Djokovic logged a 6-1 record earlier in 2026, highlighted by his Australian Open runner-up finish to Alcaraz on January 31.
